Community Nursing Care
We can assist with the provision of specialist care for participants who have high care needs requiring a high level of skill and for the training of support workers to respond to the participant’s complex needs.
Assist-Travel/Transport
Hope & Home Care Community has its own transport vehicle for many passengers or for individual use. Our licensed support workers are always ready to provide safe transportation.
Assist-Personal Activities
Personal care supports relate to assistance with daily personal activities including assistance with, or supervision of, personal tasks of daily life. This may be required across a variety of settings.
Innovative Community Participation
We assist and encourage individuals who wish to participate in the wider community. This includes- Programs at local councils, Library services, Youth group participation, Religious and Social Justice events.
Daily Tasks /
Shared Living
We provide time-limited, goal-oriented and therapy-focused packages of services to older people after a hospital stay to help recovery and re-establish independence. Services may include – Nursing support.
Domestic and
Household Tasks
We can provide assistance with essential tasks that the participant is not able to undertake such as meal preparation, and delivery, house or yard maintenance, cleaning and linen. This will be achieved through assistance.
